The following is a list of forsen's offenses that got him banned from Twitch for various lengths of time.
The BAND emote.
2016
On January 28, 2016, forsen received his first known ban on Twitch. Not much can be found on the internet whether there was a specific event that caused this aside from his own tweet:
"Banned from twitch for 24h, basically a lot of offenses adding up. Ill be back tomorrow"

As he describes it himself, it could've been caused by forsen's very lenient moderation of his channel over a longer period of time, however comments under the aforementioned post do mention a certain "dick gif" or "link" that might have been the catalyst for the 24 hour suspension of his channel.

2017
The second ban happened in January of 2017, not exactly clear on which day, though 11th is a likely answer thanks to a Reddit post from that day titled "Forsen gets a 24 hr ban because of his community was spelling "IT" in the AGDQ Twitch chat" on the r/LivestreamFail subreddit.
Not a lot can be found about this incident as no VoDs, clips or Tweets could be found. One of the main sources of information is the comments under the previously mentioned post, such as:
For everyone who doesn't undestand. Yesterday someone donated to Forsen, telling that the viewers should raid the AGDQ stream and spell "it" (meaning the n word). Forsen told his viewers not to do this, but a lot of them did it anyways.
A day after this Forsen got banned for breaking the Twitch rules.

Another two mentions are a Kotaku article released a year later mentioning the AGDQ situation and a Wikipedia article referencing the Kotaku article.

Forsen has also been banned from hosting the following year's AGDQ, explaining it via the following thread of tweets:

2020
This year was a tumultuous one for our Swedish streamer, as he received 3 bans.
The Valorant rant
On May 7, 2020, forsen was playing Val
The reason for this ban is apparently forsen's voice communication during the game, calling his teammates "retarded Russians." Twitch, of course, took this out of the game's context and swiftly banned him.
As is normal for Twitch, there was no communication about the reason for the suspension, so bajs had to wait for forsen to remember his Twitter password to learn more details.
A few days later, on the 11th, forsen posted this tweet explaining the reason for the ban:
"Banned for 2 weeks for saying retarded russians instead of retarded "people who cant participate in voice comms in valorant due to legal reasons". I guess I have some more time to beat up my half russian siblings now. nice"

The day after he has been unbanned, on May 23, 2020 he clarified the situation in a little more detail as well as sharing some of his thoughts on the situation as well as Twitch staff's double standards:

Interestingly enough, he mentions showing "mutilated cocks on stream" and that he got banned for this for a day, which could be referring to the previously explained 2016 one day ban. He's trying to count the times he's been banned, as well as someone in the chat mentioning he did have a one week ban. All these could be combination of forsen's exaggerations as well as chat's trolling or hallucinations, but there is a chance that there have been bans which are lost to time. If you have any context for this, or even better yet, any clips, tweets, conversations, please contribute in any way if possible.
The day forsen has returned is also the first day of his Minecraft adventure that, unbeknownst to both him and his chat, would leave everyone scarred forever.
The Swedish accent
The second ban of 2020 happened on September 10. This time, nobody had an actual idea what happened, not even forsen.
He was finally contacted by Twitch informing him that "You said the f-word" on stream, which he clearly did not have any memory of doing so. His janitors eventually found a clip that could have been the cause. While streaming Diabotical 5 days earlier, because of his Swedish accent, "Fuck it" or "Fäääk it", unclear which one it was, sounded like the "f-word" if you weren't used to watching forsen. This resulted in some staff at Twitch being very trigger happy and handing out the suspension.
It was originally a 7 day ban, which was reduced to 1 day following forsen's appeal.

The infamous horse incident
The third and last ban of 2020 is the most known and longest one he has ever received.
On November 26, 2020, it was a stream like any other. He was checking out the links from his donos until he opened a certain image hosted on imgur.com:

Forsen's lightning fast reaction he has trained for years that allowed him to take the image off the screen was apparently not good enough for Twitch, as he has been banned the same day. The next day, he posted a tweet revealing that it is, somewhat unexpectedly, an indefinite suspension.

This has notably caused an uproar, as not long ago Pokimane, a major normie streamer, has shown actual porn from PornHub on stream and was allowed to go with just a slap on the wrist from Twitch.

A bunch of streamers started chiming in on the situation, including "Horses don't wear clothes" from Asmoncuck:
A month later, on December 26, 2020, forsen has been released and has gone live the following day.
After small random discussions, he got to the main topic he wanted to discuss - the Financial Board Meeting, which lasted 96 minutes. He touched on a lot of issues with modern-at-the-time Twitch.
In the end, the incident has scarred him for life. Any appearance of that image triggers a PTSD and the bajs love teasing him with a lot of horse-related content, be it sound, videos, being able to show a horse to him in a game and so on. It often results in a 
2023
After the horse incident, forsen would hold a clean record for the next few years until the April 20, 2023.
It was a stream like any other. He was checking out the links from his donos- wait, I'm getting a déjà vu. I wonder why. Anyways, he was watching links as he usually does when he opened a certain video titled "Hololive ASMR - Mori Calliope Eat Horse D."
Forsen, being used to garbage like this, didn't think much of it and just let it play for a moment until he realized what he's actually listening to. As the title promised, it was an (AI generated) ASMR of a VTumor "Mori Calliope" giving a blowjob to a horse.
That's right. He lost to a horse. Twice.
Neither the stream VoD, nor the original YouTube video are available anymore, but a clip has been saved on an r/LivestreamFail mirror in a now-deleted post titled "Very average forsen normally watching a very normal video from a very normal donator":
This, unsurprisingly, resulted in a one week ban from the platform, until April 27, 2023.
Forsen has explained the reason for the ban in a tweet a few days later:
"forgot to tweet, I’m banned cause of the dubious asmr youtube video for 1 week . Unlucky"

During a stream after getting unbanned, he clarifies that a Twitch representative he has been in contact with has confirmed the reason for the suspension. He also sets new standards for links for a while to at least partially avoid this kind of tomfoolery in the near future:
After this ban, forsen has not received any suspensions as of writing this article. Knowing our favorite clown however, this is likely a subject to change in the future and will be documented should it happen.
Notable mentions
While he did not get banned for this, a warning can be as scary as a ban itself.
One such example is when forsen was watching Bad Black. He received a warning from Twitch that he should stop as the movie had a naked child in it.
Forsen Film Festivals
Of course, it would be unwise to talk about forsen and not mention the movie festivals which played in the Artifact section during his "vacations."
The whole topic deserves its own article. However, here is a short explanation.
After forsen's November 2020 ban, the bajs were exceptionally bored to the point of going to the Artifact section and streaming series and movies for each other. This became a tradition for whenever forsen was gone for longer periods of time, like his two weeks of relocation to Cyprus, though that was streamed mostly on Kick as Twitch was cracking down on DMCA content.
